SPASMODIC TORTICOLLIS - Procedure & Fee

FEES:
IMPORTANT NOTICE ! Several clients have received full insurance coverage from major national insurance providers for the clinic fee. Ask your Neurologist or Doctor for a pre-approval letter recommending your participation in the S.T.R.C. Recovery Program in New Mexico or the Long Distance Course and present that to your insurance company for possible coverage. We will also provide you with a letter detailing the CPT codes and cost breakdown. Although the Director is not a Dr., we have on our Board of Directors a Registered Nurse, Licensed Physical Therapist and Licensed Massage Therapist. S.T.R.C., Inc. is a non-profit, tax exempt 501 (c)(3) corporation.

PLAN A: New enhanced Premier Program - for a fee of $1,500.00
OR
PLAN B: Our Original Program for a fee of $800.00

Both Plans provide:
• Daily private one-on-one or two-on-one classroom instruction in the exercise program, both the stretching and the gym work
• Skills that will enable you to physically support your recovery process rather than the S.T. on a daily basis and hasten your recovery
• Motivational work (guidelines to help you work through the tough times)
• The Clinic Program Guide detailing all of the exercises and more
• Two books
• Two cassette tapes
• The viewing of two films
• A daily upper back and neck rub/massage
• A nutritional seminar with materials and free ongoing nutritional counseling with the clinic nutritional consultant if needed
• A generous supply of high quality natural vitamins
• A two year follow-up period with free Quarterly Updates
• Luncheon meals Tuesday through Friday, including a luncheon at a nice restaurant, gratis S.T.R.C.!
• Transportation from your hotel / motel to the clinic and back if needed.
• Inclusion in a former client Email Support Group, if desired

PLAN A, the Premier Program includes all the above, PLUS:
• The Instructional Photo Manual and Video
• An afternoon at a day spa, for instruction in pool exercises and Jacuzzi
• Free Physical Therapy tool
• A private, one hour full-body massage by Licensed Clinic Massage Therapist, Myra Jacob
• The Clinic Cookbook, Come & Dine, authored by Clinic Director
• Open-ended follow-up, for as long as the client wishes help, in addition to free Quarterly Updates.

We accept Master Card and Visa, personal check, money order or cashiers check.

• A spouse,relative or friend intending to help the client with his or her ongoing recovery program may accompany the client for the clinic week at no extra charge an clinic lunches at the clinic are complementary for everyone, however at the restaurant the clinic only covers client meals.

• As a non-profit organization, we seek, on an ongoing basis, financial grant assistance/ endowments for the establishment of a fee scholarship fund. Donations contributed to help support this work and the needs of fellow ST'rs are fully tax-deductible, and these gifts are greatly appreciated.

Once your application is in:
you will be mailed a Confirmation and a Santa Fe Visitor's Guide along with maps, hotel/motel listings, a map to the clinic, and a form to fill out and return to our office. Your check will not be cashed until you have arrived in Santa Fe for your clinic. Credit cards will be charged 3 days prior to the beginning of your clinic week. Anytime prior to the beginning of your clinic week, should your plans change and you find it necessary to cancel, your full payment will be returned to you.
